Two   If you or someone you know needs help recovering from an illness or injury, Wooster Community Hospital's Home Health Department is at your service. Our professional team of nurses, therapists, social workers and home health aides provides in-home care to patients and families in a Wayne, Holmes, and Ashland counties. Our goal is to offer compassionate care while helping patients regain as much independence as possible.

  1 Listed below are the services provided by team members:    
       
  1 Registered Nurses coordinate care of patients under a physician's medical direction and provide skilled nursing services.    
       
  1 Physical Therapists establish a specialized rehabilitation program for patients requiring strength, endurance and gait training.  
       
  1 Occupational Therapists provide activities and devices to assist patients in becoming more independent.
 
       
  1 Speech Therapists are available to help patients experiencing swallowing difficulties or some degree of speech loss.

 
       
  1 Certified Home Health Aides provide personal care, including bathing and grooming, and assist with light housekeeping duties.  
       
  1 Medical Social Workers coordinate resources available from local community agencies for social and financial support that may be required by our patients.  
       
  1 Enterostomal Therapist manage wound care, and also help patients who need ostomy care.  
       
  1 Support staff is also available from other departments, such as Nutrition Services, Laboratory and Respiratory Therapy.

  arrow Home Health Services are available to individuals who need skilled care due to an ongoing condition, disability or recent illness. Hospitalization is not required before receiving care. To qualify for our Home Health program, the patient must be under a physician's care and must generally be confined to their home. This means your condition does not permit leaving the home on a regular basis or leaving the home is a hardship.

A registered nurse is on-call 24 hours a day for our clients.

 
      Payment for Home Health Services is provided:  
    arrow 100% coverage for Medicare and Medicaid eligible patients  
    arrow Many private insurance companies have provisions that allow for treatment of illness in the home setting.  
    arrow Workers Compensation with prior authorization from Bureau of Workers Compensation
