Transitional Care Unit

In late 2012, we will open a new Transitional Care Unit.  Designed to accommodate and care for patients who are not ready to return home and do not qualify for Inpatient Rehabilitation, this unit and our staff will help patients recover from conditions such as pneumonia, congestive heart failure, a stroke, and other illnesses requiring the patient to have additional therapy or assistance.

The goal of the Transitional Care Unit will be to help people get past the hurdles of recovery and back to their lives, work, family, and independent living.  Patients may stay a few days or several weeks, depending on their needs.

Our professional team of nurses, therapists, social workers and home health aides provides in-home care to patients and families in Wayne, Holmes, and Ashland counties. Primary Stroke Center
Primary Stroke Center

Wooster Community Hospital has been accredited as a Primary Stroke Center by the Joint Commission. The Stroke Team at WCH rapidly evaluates and directs the treatment for all stroke patients.
