Medical Records

A patient may obtain access to a copy of their medical records by completing the "Authorization for Disclosure" form and bring it to the Health Information Management department located on the first floor of the Wooster Community Hospital, where medical records are maintained. You will also need to bring one type of identification with you, such as a driver’s license or photo ID.

Monday through Friday 8:00 AM to 4:00 PM

The Health Information Management department is located on the First Floor of the Main Building here at Wooster Community Hospital.
