Patient Care Navigator

In a hospital setting, a “Patient Navigator” is someone who provides individual assistance to patients and family members to help overcome healthcare system barriers.  This role can involve education, linking to internal or external resources, handling insurance questions, or simply facilitating timely access to care.

Wooster Community Hospital offers the assistance of two Patient Navigators at your service.  They can provide assistance with medical care as well as connecting patients to available resources.

The Patient Navigator office is on the ground floor next to the hospital auditorium, and just up the hall from the Breakaway Café coffee kiosk.

Meet Our Team

Our team includes Polly Johnson, Patient Navigator and Susan Van Sickle, RN, Oncology Specialist.  They can be reached at 330-202-5162 or by email at pjohnson@wchosp.org.

Polly previously worked for the American Cancer Society, where she was a Patient Navigator for 8-1/2 years.  She’ll work closely with patients, their families and the medical staff to make sure they are aware of all the resources available to them.

Susan has been a nurse at Wooster Community Hospital for several years, and brings her nursing experience to the Patient Navigator role.  As a nurse, she can talk to patients and their families about their medical care, support, and help with medical resources and answers.

Wooster Community Hospital offers a Home Assistance program to help you as a family member or caregiver who dedicates your time to caring for an adult loved one.
