Forms and Privacy Practices
The confidentiality of your health information is important to us. As your community hospital and health care provider of services, such as home health, we rely on you to give us complete and accurate information about your condition, symptoms and health history. We want you to know about our privacy practices that are intended to safeguard the proper use and disclosure of your health information
The Health Insurance Portability and Accountability Act of 1996 (HIPAA) established new standards for the movement and uses of health care information. There are three types of standards created by HIPAA: privacy, security and administrative simplification (e.g., transaction standards).
Taken together, these regulations have a major impact on the day-to-day functioning of the nation's hospitals and affect virtually every department of every entity that provides or pays for health care.
Every effort will be made to resolve complaints or concerns with the Department Manager. However, in accordance with the Medicare Conditions of Participation, a grievance process is available for complaints regarding quality of care or premature discharge issues that can not be resolved in a less formal manner. This process can be initiated by notifying the Patient Advocate at 330-263-8727.
Complaints may also be forwarded to the state survey agency regardless of whether or not the hospital's grievance process has been utilized
