Skilled Nursing

Skilled Nursing – Transitional Care Unit


The goal of the transitional care unit, Wooster Community Hospital’s skilled nursing and rehabilitative facility, is to help people get past the hurdles of recovery and back to their lives, work, family, and independent living. Our skilled and dedicated nursing staff are available for your loved ones 24 hours a day, 7 days a week, to ensure they get the high-quality care they deserve to make this transition as seamless as possible. Patients may stay for a few days or several weeks, depending on their needs.

5-Star Nursing Home in Wooster, OH

Our transitional care unit has received the Best Nursing Homes award from U.S. News & World Report for high ratings in the following categories:

  • Health inspections and complaints, which show how well our facility met home and safety standards for food preparation and other activities.
  • Staffing, which looks at the average number of hours per day of care, was received per resident.
  • Medical care quality measures, which identify the percentages of residents who received recommended health care, such as flu vaccinations, and those who experienced care-related problems, such as pain.
  • Fire safety, which shows how well our facility met National Fire Protection Association (NFPA) standards.

More than 15,000 nursing homes nationwide were evaluated, and our facility was the only skilled nursing facility in Wayne County, Ohio, to receive an overall rating of 5 out of 5 stars after being reviewed in all categories. The staff at Wooster Community Hospital takes this honor very seriously and continuously works hard to maintain this status year over year.

Transitional Care Unit Services

There is a wide range of services that are offered by the expert providers in our transitional care unit. These services include but are not limited to the following:

  • Physical therapy
  • Occupational therapy
  • Inpatient rehabilitation
  • Assistance with personal grooming

As with all other services provided by Wooster Community Hospital, the care, and attention provided to your loved ones in the transitional care unit is personalized to their individual needs. This means your loved one is getting the proper care and treatment they need to make the transition back to independent living as quickly and as seamlessly as possible.

If you have any questions about your loved one’s treatment plan, we encourage you to be an advocate and speak with one of our nurses on staff. They’ll be happy to listen to your concerns and alter treatment as appropriate.
